How do I apply for a lost passport certificate?

  • Please contact the Investigation Squad of any precinct. Procedure: 1.Lost domestically: Applicant shall present identification to and file application for lost and void ROC passport, a two-counterpart form, with the police station at the applicant’s domicile or place of loss. The processing police unit will register the application with the Automated Lost Passport Processing System. 10 minutes after the system has verified the passport number, the undertaking personnel will fill in the passport number and date of issuance on the said application form and affix a seal stamp exclusively for passport handling. The application form will be returned to the applicant and may be used as a certificate of lost passport. 2.Lost abroad: (1)Required documents: a. Certificate of Entry Permit issued by the consulate office of our Government b. Copy of Certificate of Entry Permit Present the documents listed above and apply for a new passport directly from the Bureau of Consular Affairs, Ministry of Foreign Affairs, or the Central, South, or East Office of the Ministry of Foreign Affairs. (2)Those who entry without Certificate of Entry Permit shall report to the Investigation Squad of county/city police precinct and present a copy of Certificate of Entry Permit. (3)Those who lost their passport in Mainland China (including Hong Kong and Macau) shall report to the Investigation Squad of county/city police precinct with proof of legal entry and complete the application form for lost ROC passport. 3.Those who lost an expired passport shall apply for a new passport directly from the Bureau of Consular Affairs, Ministry of Foreign Affairs, or the Central, South, or East Office of the Ministry of Foreign Affairs.
